How to Begin Services
Eligibility Requirements
Enrollment in an approved DDS Medicaid waiver program
Referral from your physician, DDS case manager, or provider agency
Contact your DDS case manager to request Alphacare therapy or nutrition services.
The case manager submits the referral and authorization.
Our intake team matches you with a licensed provider in your area.
Services begin at no cost to you or your caregivers.
